Privacy of your personal information
is part of our clinic's policy of providing you with quality care.
All of our team members who come in contact with your personal information
understand the importance of protecting you and are committed to
collecting, using and disclosing your personal information responsibly.
Our privacy policies and procedure comply with federal legislation
called the Personal Information Protection and Electronic Documents
Act (PIPED) which was passed January of 2004.
In this form, we have outlined what our office is doing to ensure
that only necessary information is collected about you; that we
only share your information with your consent; that storage of your
personal information complies with existing federal legislation
and privacy laws; and our privacy protocols comply with privacy
legislation and standards of the College of Chiropractics of Ontario.
Here is how our office collects, uses and discloses our patient's
information:
1. To deliver safe and efficient chiropractic care.
2. To assess your health needs.
3. To advise you of treatment options.
4. To establish and maintain contact with you.
5. To invoice for goods and services.
6. To fill out insurance and WSIB forms.
7. To defend a legal issue.
8. To collect unpaid accounts.
9. To contact you via E-mail re our newsletter events.
10. To contact you about special events or opportunities we have
available.
11. To communicate with other health care professionals about your
health if a second opinion is necessary or co-management of your
case is needed.
12. To communicate with your employer when co-management of your
case is necessary due to a work related injury.
13. To allow us to contact you to distribute health care information,
to book and confirm and reschedule appointments.
14. To comply with undertaking of governing bodies according to
the provisions of the Regulated Health Professionals Act (Reg H.A.).
15. To comply with undertakings by a regulatory body for monitoring
purposes or otherwise.
16. To deliver your records to our clinics Insurance company for
assisting on liability and to quantify damages, if any.
17. To generally comply with the law.
18. To prepare materials for Reg. H.A. & Revenue Canada (Rev
Can).
19. To allow potential purchaser of the clinic to evaluate the clinic.
20. To allow a locum Doctor access to your health information in
Dr. Gardiner's absence.
When unusual requests for your information are received, we will
contact you for permission to release such information. We may also
advise you if such a release is inappropriate.

including asking questions about the content of your charts/records,
